One ballot per person, duplicates will not be counted. All responses are confidential. Names and phone numbers are for prize notification only. Ballots must have at least 20 answers to qualify. Any perceived attempts at ballot stuffing will be disqualified. We reserve the right to reject any entry. Original ballots must be delivered no later than August 1, 2008 to qualify. No faxes or photocopies will be accepted, no exceptions. Results will appear in the September 2008 issue.
